Flying with dogs to the United States

The United States Centers for Disease Control and Prevention (CDC) have established additional requirements, effective from 1 August 2024, for the admission of all dogs into the country, regardless of whether they are travelling in the cabin, in the hold as checked baggage, and even if they are assistance dogs. More information on the new regulations is available on the CDC website (in English only).

One of the new requirements is the completion of this official form a few days before the date of travel. In the yellow box at the top of the form, you must state whether or not the dog has been in a country or region at high risk of rabies infection in the last six months. If that is the case, you will have to fill in some additional information on the form.

Once you have submitted the form, you will receive a receipt that you must show in printed or digital format at the LEVEL check-in desk both on the day of your departure flight and when you arrive in the United States, along with any other required documents for your dog.

If your dog has been in a high-risk area and you do not have a valid vaccination certificate issued in the United States, or an export health certificate approved by the USDA, it can only be admitted through one of the six airports that have CDC animal care facilities: Atlanta, Los Angeles, Washington, New York, Miami or Philadelphia. You must also pre-book your dog's stay at these facilities.
